MNYADE is a chapter of the American Association of Diabetes Educators (AADE). Members include registered nurses and dietitians, social workers, pharmacists, physicians, psychologists, podiatrists and diabetes-related industry representatives. Membership in MNYADE offers:

The form will be displayed, which you can then print and complete manually. Or, use our new On-Line Change Form and submit your changes electronically.
